Prior to beginning therapy with Charity Cortez, LCSW at Luminous Waters Psychotherapy PLLC, each client will have an opportunity to review the Practice Policies for clear information outlining telehealth practice logistics, communication, scheduling/cancellation, payment/fees, privacy/HIPAA guidelines, and expectations for therapy. All clients will be asked to provide informed consent, acknowledging their understanding and agreement to adhere to these practice policies.

Charity Cortez, LCSW is licensed to provide psychotherapy services to clients who reside in Texas.

A note about Good Faith Estimates:

Under Section 2799B-6 of the Public Health Service Act, health care providers and facilities are required to inform individuals who are not enrolled in a plan, coverage, or Federal health care program, or not seeking to file a claim with their plan or coverage, both orally and in writing, of their ability, to receive a “Good Faith Estimate” of expected charges. You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
